The U.S. Department of Health and Human Services on Monday awarded $25 million in planning grants to expand access to home and community-based services through Medicaid’s Money Follows the Person program. With these grants, 41 states and territories participate in the program benefiting older adults and people with disabilities transitioning from institutional care to their homes and communities.
